Walk Your Talk by Theresa Cheung is a useful and accessible guide to changing one’s life for the better, with actionable steps in a four-week program. Cheung adds a new habit each week (drinking more water, improving sleep by going to bed/rising at a consistent time, etc.) supported by recent scientific research. Cheung’s program encourages people to change slowly, building confidence as they do.

There is nothing here that isn't already covered in more thorough detail and research in other books on the topic. It's a nice entry point for someone at the start of a life-transformation journey, but doesn't offer much for those further along.
