If you are a librarian, the name and address of the library that you work for;
If you are a bookseller, the name and address of your bookstore;
If you are a media professional, the name and address of the company where you work;
If you are reviewer or blogger: ---the genres you prefer to review, ---the name of the publication/blog where your review will be published, ---the direct link to the publication/blog (including number of followers and page views), ---links to GoodReads and/or Amazon profile pages, ---links to any previous reviews, and ---information about your online reach (including any Facebook or Twitter pages);
If you are a member of any Professional Associations, the name of the association.
